Laboratory Hours and Appointments

Generally, lab testing can be performed on a walk-in basis and no prior appointment is required. However, if questions arise as to any type of patient preparation ( i.e. Should I come in Fasting?), the patient is encouraged to contact the lab office at (419) 479-5445.

Glucose tolerance testing (2 hr, 3 hr, etc.) are performed by appointment only. Contact the lab at (419) 479-5445. Post vas collections can be dropped off any time, but must be delivered to the lab within 8 hours of collection.

The laboratory’s Draw Station Patient Service Centers are routinely open Monday through Friday from 8:00 A.M. until 5:15 P.M.

The Draw Station Patient Service Centers are closed on major holidays and the day after Thanksgiving.

Written Doctor Orders

It is necessary that each patient have a written and signed order(s) from a physician(s) for all testing to be performed. The order can be in the form of a script or lab order form and may be brought by the patient or faxed directly to the lab at (419) 473-2510. A notation of the patient’s diagnosis(s) is required and will eliminate an additional phone inquiry to the physician’s office.

Orders can be set-up as standing orders so that patients can have the same testing done over a period of time. A standing order should not be for more than 1 year. Simply indicate that the order is a standing order on the initial order. Be sure to include any special instructions for frequency of testing and starting date.
