The Toledo Clinic Health Education Center provides comprehensive health education to patients and their families with the purpose of enabling our patients and their families to better manage their diseases and improve quality of life. We strive to provide flexible programs designed to meet the needs of our patients and their schedules. We are a diverse team of healthcare providers passionate about helping our patients get the care, education, and support they need. Our team includes: pharmacists, dietitians, registered nurses, certified diabetes educators, and a certified insulin pump trainer.
ZAPS is a program designed to educate people on how to lose weight and keep it off! This is a yearlong, CDC (Center for Disease Control and Prevention) approved program that will allow you to learn how to implement healthy lifestyle changes, as well as build upon the skills you will learn to maintain your results.
A diagnosis of diabetes is a life-altering event that many people continuously struggle with. They are told it is crucial to keep blood sugar levels under control to prevent complications such as heart attack and stroke, kidney failure, blindness, nerve damage and pain, amputations and more. Diabetes is a chronic condition that is primarily managed and controlled by the person with diabetes. To manage diabetes, it is important to fully understand the disease as well as ways to keep it under control. A management plan should include monitoring blood glucose levels, meal planning, physical activity, and stress management. In some cases, insulin or oral medications may be necessary to control levels. By managing diabetes closely, you will live a longer, healthier life and deter the complications of diabetes. For this reason, following a management plan is important and should be discussed with a physician or diabetes educator. This plan may change as your body changes over time and different treatment options become available.
The Toledo Clinic Health Education Center offers an AADE (American Association of Diabetes Educators) accredited diabetes self-management education program. We provide excellent care and service focused entirely on YOU. Our multi-disciplinary group of medical professionals, including pharmacists, dietitians, registered nurses, certified diabetes educators, and a certified insulin pump trainer, will work together with you to develop personalized diabetes action plans that suit your needs.
